The Reality in New Hampshire

Alcohol Use in New Hampshire

New Hampshire consistently ranks among the highest states for per-capita alcohol consumption in the nation. Approximately 1 in 4 NH adults report binge drinking, and alcohol-related emergency department visits continue to rise across the state.

Alcohol is the most widely used addictive substance in New Hampshire, yet only a fraction of those who need treatment actually receive it. Alcohol contributes to hundreds of deaths annually in NH through liver disease, accidents, and alcohol-related incidents.

These aren’t just statistics — they represent families in Salem, Derry, Nashua, Manchester, and across the region who need access to compassionate, evidence-based alcohol treatment.

For Family Members

How to Help a Loved One with Alcohol Addiction

Learn About Alcohol Addiction as a Medical Condition

Understand that alcohol addiction is a medical condition, not a lack of willpower. Chronic alcohol use physically changes the brain’s chemistry and reward system, making quitting without professional support extremely difficult and potentially dangerous.

Understand the Risks of Alcohol Withdrawal

Alcohol withdrawal can be medically dangerous. Unlike many other substances, severe alcohol withdrawal can cause seizures, delirium tremens, and can be life-threatening. Professional medical supervision is critical. Never encourage someone to quit “cold turkey” without clinical guidance.

Set Boundaries Without Ultimatums

Communicate clear limits: “I love you, and I won’t participate in behaviors that support your drinking.” Follow through consistently. Boundaries protect your wellbeing and can motivate change without pushing them away.

Have the Conversation — Timing Matters

Choose a time when they’re sober. Use “I” statements: “I’m worried about your health” rather than “You drink too much.” Mention that medications like Vivitrol can reduce cravings and make recovery more manageable. Offer to call Trailhead together.

Take Care of Yourself

Your mental health matters too. Consider therapy for yourself, join a support group like Al-Anon, and maintain your own routines. You can’t pour from an empty cup.

Family Support at Trailhead

Our family support program is open to all family members 18+, past and present clients. We offer individual family therapy sessions and monthly Zoom education groups covering codependency, family systems, and substance use education.

When a Loved One Won’t Seek Help

What If They Refuse Treatment?

It’s common for people struggling with alcohol addiction to resist treatment. Denial is a hallmark of alcohol use disorder — but professional treatment with medical support makes recovery achievable.

A structured, guided intervention led by a professional can break through denial and help your loved one see the impact of their drinking on themselves and the people they care about. Alcohol addiction often involves deep denial, making professional guidance especially valuable. Interventionists are trained in compassionate, non-confrontational approaches.
Rather than confrontation, express concern with empathy. Motivational interviewing techniques focus on their own reasons for wanting change — helping them arrive at the decision to seek help on their own terms.
Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific about what you will and won’t accept, and maintain those limits consistently.
People move through precontemplation, contemplation, preparation, action, and maintenance. Meeting them where they are is more effective than pushing. Recognizing which stage they’re in helps you respond appropriately.
If there’s immediate danger — seizures, confusion, loss of consciousness, or difficulty breathing — call 911 immediately. Severe alcohol withdrawal is a medical emergency. For non-emergencies, call the 988 Suicide & Crisis Lifeline. Our admissions team can also guide you through next steps and discuss intervention options.
