Signs & Symptoms of Drug Addiction

Substance use disorder ranges from mild to severe and varies by drug type. Select a severity level to learn more about the warning signs.

Early Warning Signs

  • Using drugs more often or in larger amounts than intended
  • Failed attempts to cut down or stop using a substance
  • Spending significant time obtaining, using, or recovering from drug use
  • Strong cravings or preoccupation with the next use

Escalating Patterns

  • Failing to fulfill responsibilities at work, school, or home due to drug use
  • Continued use despite relationship, legal, or financial problems caused by substances
  • Doctor shopping, borrowing medications, or obtaining drugs through deceptive means
  • Giving up important social or recreational activities to use or recover from use

Critical Indicators

  • Tolerance — needing increasingly higher doses to achieve the same effect
  • Withdrawal symptoms that vary by substance: anxiety, insomnia, agitation, tremors, paranoia, fatigue, or depression
  • Physical signs of drug use — weight loss, dilated or constricted pupils, nasal damage, skin changes, dental problems
  • Mixing multiple substances (polysubstance use), which dramatically increases health risks and overdose danger

The Reality in New Hampshire

Drug Use in New Hampshire

Drug addiction affects thousands of New Hampshire residents across all substance categories. Stimulant use — including cocaine, methamphetamine, and prescription Adderall — has surged in recent years, with stimulant-involved deaths increasing by over 50% statewide since 2020.

Benzodiazepine misuse (Xanax, Valium, Klonopin) and polysubstance use remain significant concerns, with many overdose deaths involving multiple substances. Synthetic drugs, counterfeit pills, and marijuana dependency are also growing issues across the state.

These aren’t just statistics — they represent families in Salem, Derry, Nashua, Manchester, and across the region who need access to compassionate, evidence-based drug addiction treatment.

For Family Members

How to Help a Loved One with Drug Addiction

Educate Yourself About Their Substance

Understand that drug addiction is a medical condition, not a character flaw. Different substances affect the brain differently — stimulants, benzodiazepines, and prescription drugs each carry unique risks. Learning about the specific substance helps you understand what your loved one is experiencing.

Recognize the Dangers of Polysubstance Use

Mixing substances is especially dangerous. Many people who struggle with one substance use others simultaneously. Combining stimulants with depressants, or mixing benzodiazepines with alcohol, dramatically increases the risk of medical emergencies. Be aware of signs of intoxication from multiple substances.

Set Boundaries Without Ultimatums

Communicate clear limits: “I love you, and I won’t participate in behaviors that support your drug use.” Follow through consistently. Boundaries protect your wellbeing and can motivate change without pushing them away.

Have the Conversation — Timing Matters

Choose a time when they’re not under the influence. Use “I” statements: “I’m worried about your health” rather than “You need to stop.” Let them know that outpatient treatment at Trailhead can begin within 24–48 hours. Offer to call together.

Take Care of Yourself

Your mental health matters too. Consider therapy for yourself, join a support group like Nar-Anon or Families Anonymous, and maintain your own routines. You can’t pour from an empty cup.

Family Support at Trailhead

Our family support program is open to all family members 18+, past and present clients. We offer individual family therapy sessions and monthly Zoom education groups covering codependency, family systems, and substance use education.

When a Loved One Won’t Seek Help

What If They Refuse Treatment?

It’s common for people struggling with drug addiction to resist treatment. Denial, fear of change, and the grip of active addiction can all stand in the way — but professional treatment makes recovery possible.

A structured, guided intervention led by a professional can break through denial and help your loved one see the impact of their drug use on themselves and the people they care about. With many substances, the risks escalate over time — especially with polysubstance use. Interventionists are trained in compassionate, non-confrontational approaches.
Rather than confrontation, express concern with empathy. Motivational interviewing techniques focus on their own reasons for wanting change — helping them arrive at the decision to seek help on their own terms.
Follow through on consequences you’ve communicated. Boundaries protect your wellbeing and can motivate change. Be specific about what you will and won’t accept, and maintain those limits consistently.
People move through precontemplation, contemplation, preparation, action, and maintenance. Meeting them where they are is more effective than pushing. Recognizing which stage they’re in helps you respond appropriately.
If there’s immediate danger — suspected overdose, seizures, extreme agitation, psychosis, or unresponsiveness — call 911 immediately. For non-emergencies, call the 988 Suicide & Crisis Lifeline. Our admissions team can also guide you through next steps and discuss intervention options.

Our programming combines evidence-based therapy (CBT, DBT, ACT) with holistic approaches including yoga, breathwork, meditation, and nature-based experiential activities. Individual therapy, process groups, and psychoeducation are all part of every client’s treatment plan. These approaches help build healthy coping mechanisms for sustained recovery.
